This document sets out the privacy policy of Paul & Lisa Whitman (operating under the trading name Mosman Wholistic Health Consultants) ABN 13 027 081 245 (referred to in this privacy policy as ‘we’, ‘us’, or ‘our’).

Mosman Wholistic Health Consultants is a holistic healing and pain management clinic comprised of experienced practitioners who operate as independent sole traders. 

Our goal is to provide alternative medical care that will compliment or underpin conventional medicine, slow or help manage the onset of acute pain and limited function caused by chronic pain and damage as a result of injury, surgery, genetics or disease by using the fundamental Osteopath Healing Principle “Structure Governs Function”.

We take our privacy obligations seriously and we’ve created this privacy policy to explain how we store, maintain, use and disclose personal information.

        12. information in connection with client surveys, questionnaires and promotions;

        15. any other information provided by you to us via our website or our online presence, or otherwise required by us or provided by you.
  1. HOW WE COLLECT PERSONAL INFORMATION

We use third-party practice management software products and/or services including Cliniko to collect information for administrative and recordkeeping purposes. The processing of information including the storage of patient information by the practice management software will be, in addition to these terms, subject to the terms, conditions and privacy policies of the practice management software provider and we are not liable for the security or performance of the practice management software. We reserve the right to correct, or to instruct our practice management software provider to correct, any errors or mistakes in collecting your payment.

We may also collect personal information from you when you use or access our website or our social media pages. If you are providing personal and/or sensitive information on behalf of someone else, you must have the consent of that person to provide their personal and/or sensitive information to us to be collected, used and disclosed in accordance with this privacy policy. This includes where you are an NDIS participant’s nominee, support coordinator or representative. We reserve the right to request evidence of this consent (without being under an obligation to do so).

If you are providing personal and/or sensitive information on behalf of someone under the age of 18 (Minor), you must be that Minor’s parent or legal guardian and you must provide consent for the Minor’s personal and/or sensitive information to be collected, used and disclosed in accordance with this privacy policy. 

  1. SENSITIVE INFORMATION

(Collection of sensitive information) We may collect sensitive information about you during the course of providing you the services. We will only collect this sensitive information where you consent and provide us with this information. If you consent, your sensitive information may only be used and disclosed for purposes relating to providing you with our services and for NDIS compliance.

(Types of sensitive information) The type of sensitive information we may collect includes:

        1. information relating to your racial or ethnic origin;
        2. gender;
        3. sexual orientation or practices;
        4. health information, including biometric information, mental health information, and medical history and information relating to pre-existing illness, condition or disability; 
        5. family medical history; 
        6. referring health care provider and associated referral letters; 
        7. individual health identifier numbers (where necessary); and
        8. any other sensitive information provided by you or a third party to us via our Website or online platforms, or otherwise provided by you or a third party to us. 

(Use of sensitive information) Your sensitive information will only be used for the purpose of: 

        1. providing you with our goods and services;
        2. complying with our legal obligations;
        3. sending you messages, reminders, notices, updates, security alerts, and other information requested by you; or
